Webcast: Unlocking the Value of PHM Solutions: How to Establish an ROI-Based Approach in the Wake of an Ongoing Pandemic

Webcast: Unlocking the Value of PHM Solutions: How to Establish an ROI-Based Approach in the Wake of an Ongoing Pandemic

Population health management (PHM) industry solutions focus on identifying the macro/micro population segment that’s at risk and needs care. While high-performing health plans do this very successfully, most PHM initiatives fail in bringing “last mile change.”
What’s missing? Health plans need to integrate PHM insights into clinicians’ workflow to drive evidence-based engagement strategies and help physicians leverage their trust-based patient relationships. Doing so drives behavior change and brings tangible return on investment (ROI) to PHM initiatives. This is more important now than ever in this dynamic healthcare landscape and current operating environment.
Join this session to learn how to leverage technology and analytics to establish a holistic, ROI-based PHM approach—from strategy to implementation. ZS, along with the CMO of Highmark and CEO of NextHealth Technologies, will discuss how a full digital health transformation in relation to population health is now essential. We will also provide a framework built around identification, engagement, wellness and outcome that can help health plans like yours take their members from illness to wellness.
